A medical release form must be completed for each player and for each season of play. The completed medical release form must be delivered to AGLL or your team manager before your player will be eligible to participate in any team event (e.g., practice, game, party, etc).

We get it...time's are tough, especially around the holidays. If you need financial support please contact the League President and he/she will guide you through the process. 

All volunteers who will provide a "regular service to the league" and/or "have repetitive access to, or contact with players or teams" must submit a volunteer application each calendar year. A background check must be performed by a league official and candidate cleared prior to volunteer participation. You should also consider submitting a volunteer application if you wish to be considered a regular member of AGLL (NB, player members are not regular members). Regular Members can vote to elect the League's Board of Directors as well as on matters which require a vote of the Membership.

You can register online and pay by credit card or download the form if you prefer to submit a paper sponsorship form. The sponsorship package also includes information about the sponsorship levels and benefits available.

The injury tracking form is for coaches to submit accident information to the league. This form is not used for filing a claim with Little League International.

This form must be completed by parents (if claimant is under 19 years of age) and a league official, and forwarded to Little League Headquarters within 20 days after the accident. See instructions for help completing this form.

The Request for Reimbursement form is used by board members and league members to submit a claim for funds already paid on behalf of the league.

The Request for Advance form is used by board members and league members to submit a check request for funds to be paid on behalf of the league.
