The injury tracking form is for coaches to submit accident information to the league. This form is not used for filing a claim with Little League International.
This form must be completed by parents (if claimant is under 19 years of age) and a league official, and forwarded to Little League Headquarters within 20 days after the accident. See instructions for help completing this form.

A medical release form must be completed for each player and for each season of play. The completed medical release form must be delivered to AGLL or your team manager before your player will be eligible to participate in any team event (e.g., practice, game, party, etc).
Join the Booster Club–a parent/player support program–by sending a positive note to your player(s). The donation is only $5 per player. You can include a personalized message (30 words or less) to your player which will be included in the program. The free full color program includes team & player photos and highlights for the entire league which memorialize your player's personal growth, fun and friendships from this season.
All volunteers who will provide a "regular service to the league" and/or "have repetitive access to, or contact with players or teams" must submit a volunteer application each calendar year. A background check must be performed by a league official and candidate cleared prior to volunteer participation. You should also consider submitting a volunteer application if you wish to be considered a regular member of AGLL (NB, player members are not regular members). Regular Members can vote to elect the League's Board of Directors as well as on matters which require a vote of the Membership.
